All reports must be completed using official LSCSO templates only. Templates must never be altered, reformatted, or reorganized.

GENERAL REPORT REQUIREMENTS

All reports must include:
• Accurate date, time, and location
• Correct statutory charge titles
• Proper legal articulation (RS, PC, etc.)
• Chronological narrative
• Officer identification per LSCSO roster format
• Statement of attestation

Failure to include required elements may result in report rejection or court suppression.

NARRATIVE STANDARDS

Narratives must clearly explain:
• Initial reason for contact or enforcement
• Observations made by the deputy
• Legal threshold met (RS or PC)
• Actions taken
• Evidence located and seized
• Final disposition

Narratives must be factual, objective, and free of opinion or speculation.

ARREST REPORTS

Required when an individual is taken into custody.

Must include:
• Full charge title(s)
• Fine and sentencing information
• Plea (if applicable)
• Items seized from the person
• Detainee rights statement (if not otherwise documented)

EVIDENCE REPORTS

Required whenever evidence is seized, logged, or transferred.

Must include:
• Evidence type
• Location of collection
• Date and time of seizure
• Connection to the offense
• Lawful basis for seizure

Chain of custody must be maintained at all times.

TRAFFIC STOP REPORTS

Required for citations or documented warnings.

Must include:
• Reason for stop
• Observed violation
• Statutory reference
• Fine and license points

Reasonable suspicion must be articulated clearly from the outset.
